Norwood 4 Hair Transplants

What is Norwood 4?

At the Norwood 4 Stage recession of the triangular or wedge-shaped pattern in the temporal areas (front corners) becomes progressively severe as compared to Norwood 3 and a general recession occurs in the frontal region. Hair loss in the vertex (crown) increases to form a bald spot more with heightened definition over Norwood 3 Vertex. A thick brand or bridge of hair divides the temporal areas (front corners) and vertex (crown).

What to do About Norwood Stage 4?

If you have a family history of hair loss, it’s important to monitor for signs of thinning or a receding hairline, regardless of your Norwood Scale stage. Norwood 4 is a more advanced stage of hair loss compared stages 3 and 2 on the Norwood-Hamilton scale, however, it is still manageable and treatable. Treatments like hair transplants (FUT or FUE), and Low-Level Laser Therapy can help slow hair loss or restore hair.

Frequently Asked Questions

In Norwood 3, recession is mainly at the temples. In Norwood 4, both the frontal hairline and the crown are affected, with more pronounced thinning or an obvious bald spot.

Full reversal is unlikely, but treatment can improve density, slow progression, and sometimes partially regrow hair in both the frontal and crown areas.

Without treatment, many individuals continue progressing on the Norwood Scale due to genetic susceptibility, early onset, and higher follicular sensitivity to DHT. With effective medical therapies and biologics like exosomes, however, progression can often be slowed or stabilized for years. Some men naturally stop at Norwood 4 because fewer of their follicles are genetically vulnerable, allowing miniaturization to slow or plateau over time.

Those with early-onset thinning, a family history of advanced hair loss, or visible miniaturization are more likely to progress to Norwood 4–7, though treatments like finasteride and minoxidil can greatly reduce the chance of further progression.

Many people at this stage are good candidates, depending on donor density, hair characteristics, age, and whether the hair loss has stabilized.

Mainly androgenetic alopecia, driven by DHT sensitivity and genetics. Other factors (stress, health, medications) can influence shedding but are not considered primary causes.

Common options include finasteride, oral or topical minoxidil, exosomes therapy, low-level laser therapy, and hair transplantation for restoration.

Yes, Norwood 4 is visible to others. Recession plus crown thinning typically makes this stage clearly noticeable.

The progression from Norwood 4 can vary by person. Some remain in this stage for years with treatment, while others can advance rapidly over 1–5 years without intervention.

Strategic styles can help conceal hair loss during this stage, but only slightly. Longer top hair, textured cuts, or comb-forward looks may reduce visibility, but the crown remains harder to conceal.

Estimates range 1,800–3,000 grafts, depending on hairline design and crown coverage. A hair transplant surgeon must assess individually.
