Natural Transplants Procedure & Service Policy

Here at Natural Transplants (“NT”) we believe in building honest relationships with our patients. In order to ensure all patients have a smooth and uninterrupted procedure or service day, NT has instituted a set of policies (called, “Procedure & Service Policy”). Photo ID Requirement

On patients’ procedure or service date, patient must submit a photo ID to NT for patient’s file. Acceptable forms of photo ID include; driver’s license or identification card; passport; military identification; or, other similar ID issued by a governmental agency and used to verify identity.

Procedure & Service Deposit

A deposit of FIVE HUNDRED DOLLARS ($500) is due at the time of booking, in order to hold patients’ procedure or service date. This deposit (called, “Procedure & Service Deposit”) shall be applied to the patients’ procedure or service cost. If patient re-scheduling or cancellation occurs, however, THIRTEEN (13) days or less prior to a scheduled procedure or service date, the re-scheduling or cancellation terms below shall apply.

Full Payment

Payment in full, regardless of payment method, is due FOURTEEN (14) days prior to patient’s’ procedure or service date. If full payment is not received FOURTEEN (14) days prior to patients’ procedure or service date, NT will cancel patient’s’ procedure or service and patient’s’ deposit shall be forfeited to NT. Patients may call 844-327-4247 or 954-372-8824 to process payment by phone or for alternative payment instructions.

Re-Scheduling

If patient requests to re-schedule the procedure or service FOURTEEN (14) days or more prior to patient’s procedure or service, NT will re-schedule patient’s procedure or service at a cost of FIVE HUNDRED DOLLARS ($500), per request, which shall be applied to the patients’ Procedure & Service Deposit. After two (2) re-schedules by a patient, however, NT may cancel patients’ procedure or service and refuse further service with patient; in which case the Procedure & Service Deposit will be forfeited to NT.

If patient requests to re-schedule the procedure or service date THIRTEEN (13) days or less prior to patient’s procedure or service date NT may cancel patient’s procedure or service and refuse further service with patient; in which case NT shall keep the greater of (i) the FIVE HUNDRED DOLLARS ($500) Procedure & Service Deposit, or (ii) FIFTY PERCENT (50%) of the full payment.

Cancellations

If patient cancels procedure or service date THIRTEEN (13) days or less prior to patient’s procedure or service date, NT shall keep the greater of (i) the FIVE HUNDRED DOLLARS ($500) Procedure & Service Deposit, or (ii) FIFTY PERCENT (50%) of the full payment.

Refunds

NT has a strict refund policy. No refunds will be granted unless specified in this Procedure & Service Policy document. If a refund is due to a patient, however, NT requires a minimum of FOURTEEN (14) days to process the credit card or check refund.

Refund Exception

In an extreme situation, NT may waive its strict refund policies. Waiver of such policies is at NT’s sole discretion. These extreme situations are limited to the following: (i) death of an immediate family member; and, (ii) personal injury or illness – does not apply to medical situations resulting from a pre-existing condition of which the patient had knowledge of when patient booked procedure or service date, and, a signed note from the doctor treating patient for the injury or illness must be submitted to NT.
